Hello everyone, I am a pharmacy student and I am doing a job on type 2 diabetes. I have to do several surveys to different patients and I would be grateful if someone answers some questions.

The survey is as follows:
Age:
Sex: Man / woman
At what age did the disease be diagnosed?
What medication does it take for diabetes control?
Have you ever modified you?In a positive case, how?
Do you worry your illness?But
Did you change your life habits when the disease was diagnosed?In a positive case, how?
Do you know what the recommended life habits are for patients with your disease?
Is routine controls of glucose levels?But
Do you know what your current values ​​are?
Do you know what normal blood glucose values ​​are?And those of HB1AC?
