Hyperglycemia after dinner

Hello chic@s, to see if you know where I have the problem, 5 years ago I debuted with type I diabetes and lately I find it very difficultThere is no way, 2 hours after dinner I have the glucose at 250-300, I have tried to modify my fast insulin guideline (I put me 5 and I have tried with 6, 7, with 5 and 4) and the levels are still high.
The guideline I follow is: Apidra 4-5-5
Lantus 12

Before eating sympre I am well, between 89 - 100, do you think should modify the lantus?I don't know what to do and the truth is that I have little experience with modifying the lantus because I had always led little.

I never correct it because sometimes it gives me hypoglycemia at 3 or 4 in the morning and I don't know what to do.It may be that I about Lantus, before I was carrying less and my doctor changed the guideline a couple of months ago to "balance" the fast and slow insulin that uses.I will continue making changes and see what happens.
