The active patient program advances and has already begun to bear fruit.The first active patients have begun to train other patients, in this case in diabetes.On Friday, October 18, the Group Diabetological Education Workshop was presented at the Health Center of Sant Antoni that will be held by active trainers and this Monday has been held at the Can Misses health center.

With this presentation and the realization of the diabetological education workshop between peers, another step is taken in the active patient program of the Balearic Islands.Program that is included in the chronicity strategy of the health area.This session will progressively be taught in all ASEF health centers with the aim of making health professionals known the evolution of the program and the role of active -trained active patients in group health education workshops.

The health area of ​​Ibiza and Formentera launched the active patient program in February 2018. The nurses responsible for the active patient program in the Balearic Islands, Lucía Moreno and Yolanda Cáceres, accompanied by two active patients from Palma de Mallorca, started in theCAN MISSES HOSPITAL The training of the group of professionals that would be in charge in the future of training the active patients of the Health Area of ​​Ibiza and Formentera: the nurses Isabel Perelló Martínez, of the C.S.It is Viver, Mari Carmen Escandell Marí, of C.S.San Antonio and Cati Escandell Marí, Nursing Coordinator of the C.S.Can Misses.

This process, which has encompassed three levels of training, which professionals receive, the one aimed at active patients (who lead groups of other patients) and finally the one oriented to other patients, culminated in the creation of the first group of the program of the programActive patient, formed by 11 patients who were captured from all the health centers of Ibiza and Formentera: José Torres Torres, Asunción Fresneda Cobos, Antonio Guasch Escandell, Cristina Tur, José Luis Riera Hernández, María Lourdes Giráldez González, Mónica Ruiz Porlán Porlán Porlán, Narciso Cabanellas Becerra and Rafael Oliva Pericas.

The active patient program is aimed at patients with chronic health problems (which are more than 50% of the hospitalization activity and almost 80% of consultations in primary care), their relatives and caregivers, caregivers,

The objective is to train active patients who motivate and train other people with the same disease, since they start from their personal experience.In this way, empowered patients are able to make informed decisions about the treatment of their illness, consider different options to manage their conditions and promote their own health, implying in support of caregivers.

These workshops are developed through a health education strategy based on active pedagogy (that is, in investigating their own reality and the concrete situation of each), incorporating the methodology based on the same and, therefore,, motivating on the one hand the trainers to facilitate communication and exchange of experiences, and on the other by promoting that patients are more capable when making decisions, solving problems and self -managing their health problem.

The work of various patient associations has been crucial to contact all those patients who are part of the project, who were previously interviewed and an initial health assessment was made.
